In order for Dr. Bangert to have all the information necessary to advise you on your eye and vision needs, please print and fill out the above form and bring with you to your appointment.

Dr. Bangert offers comprehensive eye exams and contact lens fitting Mondays from 1 pm - 6:30 pm and Thursdays from 10 am - 2 pm.

The exams take approximately 45 minutes to complete. You will not be "pre-screened" by a technician, the exams are done completely by the doctor.

If you are a new contact lens wearer, in addition to being fitted for contact lenses, you will receive a class on how to insert and care for your lenses.
